Another series of accidents occurred on the Cipularang Toll Road, West Java, on Tuesday (2/12) involving 10 vehicles at once and killing one person, and several others were injured.The cause of the unfortunate incident this time is
brake failure
.
From the official police report and information in the field, the incident was triggered by a Fuso truck with plate number Z 9256 HA which experienced braking problems.

The truck driven by a man named Riyan Handrian Darmawan (45) was driving from Bandung to Jakarta, but could not stop when the flow of vehicles in front of him slowed down.
“My car couldn’t brake. So, that was what triggered my vehicle’s accident. Indeed, the vehicle lost control, that’s why I threw it into the left lane. But, there was no emergency lane,” said Riyan, quoting the National Police Traffic Corps website, Wednesday (3/12).

Kainduk PJR Cipularang Toll Korlantas Polri Commissioner Joko Prihantono explained that the driver had tried to steer the vehicle to the left side, but was unable to stop the truck.
The result was a series of collisions involving nine other vehicles.The heavy impact caused several vehicles to be crushed and dragged, then triggered a long traffic jam up to KM 115.
“The hard impact caused several vehicles to be wrecked and dragged, triggering a long traffic jam up to KM 115. The truck’s attempt to shift to the left side did not stop the truck’s speed, resulting in a series of collisions with other vehicles,” explained Joko.
Cimahi Police Traffic Unit Head AKP Yudha confirmed that brake failure was the main cause of the accident.
“This was a series of accidents because the driver could not control the speed of the vehicle. The investigation is still ongoing for those who died,” said Yudha.
The series of vehicles involved in this incident included Toyota Cayla B 1387 HZI, Toyota Avanza D 1074 AFP, Honda HR-V, Toyota Innova B 1819 RYB, Toyota Innova D 1144 AJZ, Toyota Innova T 1608 AG, Hiace City Trans B 7295 SAA, and Toyota Raize D 1279 AKU.
Then two goods vehicles, namely the Fuso Z 9256 HA truck which triggered the incident and the D 8753 XZ box truck.
